Updated
Highland Beach is a small town on the barrier island between Boca Raton and Delray Beach, stretched along about three miles of State Road A1A with the ocean on one side and the Intracoastal Waterway on the other. It became a town in 1949, when 21 voting residents incorporated it as a water district after salt water got into their wells and neither neighbor would supply them. The name comes from the ground itself: the land sits about 20 to 25 feet above high tide, high for this part of the coast.
The town is almost entirely residential, a mix of oceanfront condominium buildings and large private homes. The Town of Highland Beach’s planning data estimated 4,287 permanent residents in 2024, plus about 3,564 seasonal residents.
Life along A1A
Highland Beach’s three miles of sand are also nesting grounds for sea turtles. From March 1 through October 31, the town asks beachfront residents to keep their lights off from dusk to dawn, and volunteers mark nests along the shore. For many residents, the year has two seasons: the quieter months, and the winter, when neighbors return from homes up north. Keeping medical care consistent across both is often the hard part.

The physician at the Boca Raton office
Highland Beach members are seen in Boca Raton by Dr. Divino D’Alessio, MD, CAQSM. His board certification is in family medicine, and his certificate of added qualification is in sports medicine. He has worked as a team physician for NCAA Division I athletes at Coastal Carolina University, and his clinical interests include musculoskeletal ultrasound, ultrasound-guided procedures, and concussion management. Day to day, he is a primary care doctor: checkups, medications, lab results, and the questions in between.
Dr. D’Alessio practices as part of the physician team assembled by AnswersMD founder Douglas Shapiro, DO, with a shared commitment to personal, unhurried care.
Care for year-round and seasonal residents
AnswersMD cares for whole families; at the Boca Raton office, that means patients age 6 and up. In a town with thousands of seasonal residents, membership is set up so your care can travel with you:
- Your physician’s cell number, for calls and texts, with members able to reach the practice 24/7.
- Video visits from wherever you spend the rest of the year.
- Specialist coordination across two homes, with referrals arranged and followed up.
- Same-day or next-day appointments during your months on A1A.
- Unhurried visits, as long as you need, because the patient panel is kept small.
- Prevention and longevity care, with executive physicals and advanced early-detection testing chosen together.
House calls are available on a case-by-case basis.
If you’re admitted to a hospital
Your AnswersMD physician stays involved. Whenever possible, that means a visit at the hospital, conversations with the doctors treating you, an eye on your care, and someone advocating for you. The hospital team remains in charge of treatment, and your physician makes sure they know your history.
At AnswersMD, members are cared for only by board-certified physicians. You see your physician at every visit, not a nurse practitioner or physician assistant.
